Duplicate Key records

Hello,

My database has a basic, one-field primary key set on the 'Patient ID' field
(Required is set to 'Yes(No Duplicates'). My database also has some code
that autopopulates this field based on the values in the active record of the
table as shown in the 'initial form' of the series, as you progress thru the
database's series of forms. From the initial form, once it is filled out,
the user is given then choice for 'Data Entry' or 'Quality Control' as he
moves to the next form in the series. If 'Quality Control' is chosen, then
the existing record is shown in the form the user progresses to, but if 'Data
Entry' is chosen, then a blank form comes up w/ the 'Patient ID' field
autopopulated. If there is already a record for a patient, Access is
allowing this form to be filled out without any error message to the data
entry person, even though it is a duplicate key field. The good news is that
the duplicate record is not being saved, but the bad news is that the user is
not being notified that this is a duplicate key field. Why might this be
happening?
