Duplicate tasks in PWA

I have several cases in PWA where resources have duplicate tasks in their
Tasks View. The task exists once in my Project Plan. When the resource
enters time on one of the duplicates, and submits to me for update, I don't
see the update in my Updates list, and the time does not appear in the
actuals for the task. This leads me to believe the task is a 'phantom' one,
and is not connected to anything in my project.
Has this happened to anyone else? Do you know what might have caused it,
and what I need to fix it? For now, I've asked the team member to hide the
task from their view so they know the correct task to use.
Any help is appreciated

The most common source for this problem is the use of Master Projects.

Are you using them?

If you are (and I strongly [passionately] recommend not), you must never Never
NEVER publish them. If you do, you will end up with this problem.

No, I'm not using Master Projects.

Several weeks ago I had a corrupt Project file (MS Project didn't close
properly on my machine), and I had to do a restore. Are you aware if this is
a possible reason?

The most common source for this problem is the use of Master Projects.

Are you using them?

If you are (and I strongly [passionately] recommend not), you must never Never
NEVER publish them. If you do, you will end up with this problem.

Did you used restorep utility from Microsoft or something else.

Yes, a restore could cause this problem.

Please describe the process you used to restore your project.

The server support people did the restore. The SQL Server database that
contains all project files is backed up nightly using utilities. The
database was restored to a version taken 2 days earlier using SQL restore
utilities.
