Duplicate tasks in PWA

For one resource, there are some duplicate tasks in a projects in his Tasks
view in PWA. Whereas the tasks exists once in the project plan.

1. How can I eliminate the duplicate tasks?
2. Is there a way to prevent the duplicate task name creation in a project?

Any help is appreciated
