Duplicated Task Name

I have a duplicated Task Name under one of the projects.
Where do I have to go to check this problem and how do I
remove it ?

If I open the Project with Project Professional, it only
shows one task name, it doesn't have duplicated name in
there.

Are all of the task names duplicated?

Based on the limited information provided, there may be
two potential causes:

(1) You may have external links that are showing up
in the table. Turn this feature off from Tools, Options.
(2) You may have the Project summary task enabled.
Turn this off from Tools, Options.

No, there is only one task that has the duplicated name.

I have checked the two locations you have mentioned and
both are unchecked.
When we tried to update the tasks it showed up a dialog
saying.
"Some approved task changes cannot be updated in Project
files. Click view details for more information. If you
rejected a task, the resource has been notified by e-mail.

Please save your Project files with the new updates. The
updated tasks will remain in the table until you save the
project files."

If click on View Detail, it said:
"1. Administrative Time: Unknown error."

The duplicated name is under the Administrative Time
Project.

Then I clicked on OK button, the dialog disappears.

I have noticed that everytimes when I open a project from
project server, using MS Project Professional somehow the
project name changes to MSPJXXXX.MPM.
Where XXXX = 0001, 0002, 0003, and so on.
