Dynamically adding an image to a new document (only add when created)

Hi All

I am new to manipulating office document/spreadsheet so please bear
with me.
The following will also apply to an Excel spreadsheet.

I have a template document which will be used as the basis to create
a
number of similar documents. The only difference between each
document
will be a number of images loaded at the time the document is first
created.
In other words

A use will create a new document from a .dot template file.
When the document is loaded the first time (during creation) I want to
load an image
into the document from the users local drive (each user will have a
unique image in a
given directory on their machine). However, the document may move
between users
and when each user opens the document I do not want the original image
to be replaced.

Can anyone please tell me how to do this and if possible point me in
the direction of
some decent examples.
