e-mail merge

I'm trying to do a e-mail merge and have a templete form
attached that I need people to fill out and e-mail back.
But when I send it the form does not stay intact and you
can not fill out by clicking the boxes or typing in the
fields. infact they are gone (the check box's) is this
possible

The merge document itself cannot be a form, no. The form
would have to be an attachment to the email, and that's
something Word's mail merge does not support.

Doug Robbins has posted a macro solution at word.mvps.org
you could use. Or you'd need to get a different product;
you'll find a list of alternative applications for email
merging at www.slipstick.com
