E-mailed form only displaying signature

Greetings all,

I've created a form based on the message form. It just has date and text
boxes.

I have saved and published it.

When I fill in all the fields and then send it all I'm receiving is the
signature box (duplicated many times I might add) but none of the boxes I've
created.

This was created in Outlook 2003 on an Corporate Exchange server and the
form was saved to a private folder whilst being tested.

Where have I gone wrong please?
