Easy Question: How to Begin Creating Forms

In outlook 97/98 creating forms was easy. With 2003 I just want to create a
simple form that is emailed when complete. Users would simply hit send, the
To: block will have a predefined receipent. Can anyone tell me how I create
forms in Outlook 2003, do I doownload a outlook software package and if so
where can I find it?

In 97/98 one would just typically create by slecting File, New Form,
Compose. Modify form in design few and publish. I don't see anything like
this in 2003.

Sue,

That worked thanks. One more question. When I create the new form from a
template "Message" only the text typed in the message is sent and not any of
the drop downs I've created. Any clue why drop down values are not sent?

Did you bind your controls to Outlook fields so Outlook has a place to store the data? Did you edit the separate read layout?
