Easy way to use a file to control replacements

I find I often need to perform the same series of replacements in word
documents. I actually have about 120 unique items that I need to
replace. I have a table with the replacements. As a example every time
that 192.145.3.101 appears I want to change it to NSWKS01. Is their
anyway to automate this in such a way that it is table driven or a
series a macro within Word?
