Email form for input

I need to create a short survey for some of our clients. I'd like them to be
able to click on Yes/No boxes for the questions in the survey and easily be
able to e-mail it back to me. I am not sure what E-mail software the
recipients of the survey will be using.

I orignally created a form in Word that they could click on Checkboxes for
Y/N type answers. However I do not want them to have to know how to save an
attachment or send an attachment back to me.

At this time I am not concerned about having automatic compiling of the
results, although it is something I may think about for future projects. For
this time I just want something fairly quick to create but easy for the
recipient to send back the results. Preferably some time of checkboxes
directly within the e-mail.

Although I am not new to Word, Excel, Access or VBA, I am fairly new to
Outlook and am not aware of all it's featuers.
