Email Form using VBA

I have a WORD 2003 template which is basically a form that my
supervisors will be filling out as each shift progresses. It has about
25 Form Fields on it of various types (text, number, dates). I would
like the form to be emailed to me and I would like to preserve the format.

What I've got so far is an email that has the information but all the
formatting is lost.

Is what I'm attempting even possible?

I would rather not have to open up the attachments to see the data from
the form.
