I suggest that Outlook pop-up a dialog box "You are not at the top of this
e-mail chain--are you sure you want to proceed to responded to a possibly
out-dated e-mail?" This suggest that this could be implemented/configurable
to pop-up whenever the newer emails (in your inbox) that are part of the
chain are unread or always.
