Email sent to wrong contact

One of our users (using Outlook 2003) sent an email to one of his contacts
which he selected using autocomplete. The address and display name appeared
correctly in the new message window. However, after the email was sent, he
received a response to it from a different contact. The original, intended
contact did not receive the message.
We sent another email to the intended contact and again, the other contact
received it. In his "sent items" folder, it shows the other contact rather
then the intended contact.
To fix, I deleted the contact from the autocomplete window and sent an email
by selecting his name directly from the address book. That worked.
This is not the first time this has happened so I'm wondering if there's a
way to check or some other resolution to avoid it happening again.

Two possibilities: The wrong entry was selected, or the autocompletion entry
was corrupt/outdated.
The address in Sent Items is the address that was actually selected.

I replicated the email myself more than once. Each time, it appeared correct
(name and email address) prior to hitting the send button. When I checked
the sent items, both the name and email address were consistently of the
other contact.
The two contacts are not even remotely connected. Names are completely
different as are the email addresses. They don't even work at the same
company.

Just wondering if there was ever a solution to this problem or if you could send me a link to the other topic that may have been started?

I have the same problem with an end user in my office. Our setup is Exchange Server 2003 SBS and the end user has OL 2007. This is a laptop user that is setup in cached exchange mode.

The symptoms are the same: Create a new email, click the To: button and choose recipient from the Personal Contacts. Email address in the to field is correct. Hit send. Go to the sent folder and the email address is another user from the contact folder that has nothing to do with the first email. No similar names or email addresses. This problem can be replicated as many times as I want with this particular email address. Same problem happens with other addresses in the contact folder. Most messages go without any problems.

Searching for a solution.
