Email Specific Fields from a Record

Hello All,

Can anyone help me? I am trying to use a lookup table
for my email addresses and then be able to send an email
using specified fields from a specific record. For
example, if I have a customer table. I would like to email
the salesrep the customers name, order date, and date. I
would like to be able to do this straight from a form and
not have to create a report.
