Emailing a survey in word

I've created a form in word that will be used as a survey for a mass emailing
in outlook, containing checkboxes and blanks. I'd like to have this document
appear in the body of an email so that when a recipient recieves the survey
they only need to hit reply, fill out the survey via the provided checkboxes
and fill in the blanks and hit send. Everytime I protect the document and
send a test the survey that arrives in the inbox does not have checkboxes or
blanks. I do not want to send this survey as an attachment, is there a way
to preserve the integrity of my document while using outlook to send/receive?
