Employee Evaluation with more than 5 ratings (10-15)

I am looking for performance reviews or employee evaluations or year-end
reviews that have more than 5 choices of ratings, one with at least 9 or 10
options.

More details on the options we currently use:
Consistently Exceeds Expectations
Consistently Meets and Occasionally Exceeds Expectations
Consistently Meets Expectations
Occasionally, but not Consistently Meets Expectations
Seldom Meets Expectations

I feel these are very limiting.
