empty form feilds

Hello every one:

My problem is that the form i design when the data entrey emplyee open
the form the old data appears so they edit the old one with new and
this causes duplication and many problems so i want a way so that when
employee opens the form the feilds be empty in order to avoid any
problems please help me??????????????
