Is it correct to assume that this is a non-North American setup
(specifically - in your regional settings - is the list separator character
a semi-colon?).
Short answer to your question is that the CSV file must use the same "field
separator" value as the Outlook import process is expecting. MS Office
programs, for instance, create CSV files using the list separator character
found in your regional settings - making the term "CSV" file in those cases
a <misnomer>. Not sure about the Hotmail export process and whether or not
it takes any regional settings into account.
If it's not an issue with the separator character then the other question
is - did you manually verify the Outlook import field map or just assume
that the Outlook <auto-map> will do everything correctly? If the CSV fields
are not mapped to the correct Outlook fields - nothing will get imported
except empty data. (during import process - click on <Map Custom Fields>
when that button shows itself)
