Enquiry form without Server Extensions

I use FP 2003. I'm setting up a site for some friends but they don't have a
FP Server Extensions option with their existing host and would like an
enquiry form on their site. What is the simplest way for me to create a form
in these circumstances. I've created forms previously using FP when Server
Extensions are available but I'm a bit stuck now they aren't.
Could someone point me in the right direction please.

Have them contact their host to see what they *will* support.
