Can happen if Word crashes while you are in the middle of editing a
document that contains equations, and Word autorecovers the file when
you restart Word. During the autorecovery process, the OLE links to the
equation objects become broken, and the equation objects are converted
to picture objects. Can't be undone automatically... you have to
manually reinsert each equation.

It is not a recovery from this problem, but I have discovered that I can
open an older backup copy of the document that has editable equations and
copy/paste the equations from the older version. They can be edited after
pasting. It's easier than having to retype the whole equation.
