Error creating Issues on Project Server 2002

Afternoon,

I am currently having problems adding or deleting issues
on Project Server 2002. The error message I get is:

"An error occurred in updating the Microsoft Project
Server Database. Log on to Microsoft Project Web Access
and link the document to tasks in the Task Page".

Anyone have any ideas to why this is happening???
