I forwarded your message to senior developers in my team, and one of them
quickly got back, suspecting that a mild "form/property corruption" might be
going on. Did you just recently install other Outlook Add-ins, or were you
trying to modify form customization? If you did, resetting your form cache
might just work here.
Follow these steps to reset your form cache (don't worry, it won't hurt you):
1. In Outlook, go to Tools menu > Options.
2. Choose "Other" tab, then "Advanced Options..."
3. Click "Custom Forms..."
4. Click "Manage Forms..."
5. Click the "Clear Cache" button, restart Outlook and try again.
(I don't have Outlook 2003 right now, so I wrote this based on Outlook 2007;
I would not expect them to be too much different between 2003 and 2007,
though. If the layout is different and my instruction does not match with
what you see, please look around... all should be near-by.)
Please try this and let me know if it works for you. If not, we can try to
get log file from you and see if we can find the error.

field to me. Are you trying to set the "File As" field with any specific
way? Can you create any simple contact such as "Joe Brown" and see if the
"File As" field can be saved as "Brown, Joe"?
I have not seen this before.. might need to test around to find the issue.
