Error on form

My business has an Access database used by several
people. Both the front end and back end (actual data
tables) are on our network.
We've started getting an error (when someone tries to
enter a new record) that the change can't be accepted
because it would create duplicates in the index, primary
key or relationship.
Any ideas
(I'll admit to being a novice at this}.

Do you know what the primary key is on this form and if it is an
autonumber or is being entered by the user?
