Hi Guys,
I use Access 2000.
I was asked to make an excel spreadsheet or a database to track preceptor
evaluations. There are 6 items/questions which the student will circle the
best answer and they can also add free text comments for each question (I
doubt comments will be written in but that's what they want on the
evaluation).
The choices for these questions are .... Very Poor = 1, Poor = 2, Good = 3,
and Very Good = 4.
In addition to these 6 questions, there 3 Free text fillin lines.
I found a survey database

Hookum (At your Survey 2000) and am currently looking through that and
reading the instructions to see if it will work. I need to get this done in
2 weeks and have a limited amt of time each day to dedicate to it. Has
anybody used this? Would it work for what I am doing? Is there something
else that is geared specifically for evaluations. Below is a sample of 2
questions and 1 free text questions.
The users of the database will print out reports for the preceptors so they
can see the accumulation of results and comments for a period of time.
Additionally, preceptors will be getting coached if an average item/question
total results in less than 3 or if a total of 3 evaluations average less
than 3.
1) Communicates well with all allied health personnel (Choices are above) +
Comments:_____________
2) Knowledgeable when answering questions (Choices are above)+
Comments:__________________
