Excel 2007 Pivot Table Formatting Refresh

I know this has been asked everywhere, but there does not seem to be a
defentive answer. Can you help?

1. I have a Pivot Table with formatting (Fills, Borders, Number
Format). I also have specific row heights. When I refresh, the row
heights and SOME cell fills are lost.

2. How can you hide "(Blank)" columns?
