Excel forms

Is there a way to create a form in Excel that displays radio buttons that
then indicate a choice.
I am looking for a simple way to create a student test app without having to
use Visual Basic.NET, just excel.

Any directions to point me? I have done a lot of macros, etc. when I was
working but don't know what 'form' type controls have been created for Excel
in recent times.
