Export data from custom outlook form into MS SQL Server DB

I am newbie to outlook programming. How can I get data from custo
outlook form and load it into MS SQL server DB? Any suggestions, cod
examples will be greatly appreciated
Is it possible to achieve my task using ActiveX script in MS SQL DT
package?
