extra spaces between paragraphs and dialogue changes

I have my spacing set at double and I am typing in
Courier or Times New Roman. I keep getting extra spaces
between my paragraphs and lines of dialogue. How do I get
it all evenly double spaced? I have tried selecting all
and applying double, but it still formats this way.
I have tried "keep lines together," selecting and double
spacing, etc. It's making me crazy.
