fatal error on reverse engineer

I am able to see the database tables, stored procs etc while doing reverse
engineering and when i press finish, it gives a popup error,

fatal error encountered while doing reverse engineering.

I am connecting to sybase ASE 4.5 using visio 2003. Any help is appreciated.

I have received a similar error when attempting to access an Oracle Database.
My error states:

Encountered a ata Error during reverse engineering of information from the
document.

Is this perhaps a database issue?
