Fax cover from contact

I wish to create a button on the top of each contact that when pressed would
open up a fax cover sheet with the users address/contact info and that
particular contacts info.

The user has been using ACT! for years and is accustomed to this function.

Has this been done? Is there some site or some script that I could follow to
create this?
Thanks in advance for any advice no matter how trivial!
