Faxing A Word 2000 Form In Outlook 2003

In Outlook 2003, attaching and faxing a Word 2000 document [form] with
checked formfields. When the recipient receives the fax, the check
formfields are blank. Is there anything I can do about this?

If you print the document, does it look the way you expect it to? Remember that the process of sending a fax is essentially the same as printing -- the document has to be rendered.

Then the problem seems likely to be with the fax software. You didn't mention what you're using.
