Feed Back Form

I have made a feed back form for a friends web site and every time somebody
puts a comment in the form is sends it to the recipient via my e mail
address My e mail address is included in the form from me to the
recipient.
How can I get rid of this and how did it occur as I have not placed my e
mail address on front page ?
