Field and footnote-marker "ghosts"

Sometimes, when I delete cross-reference or a footnote, it remains as an
invisible "ghost," as evidenced a missing footnote number, and when I browse
by field or by footnote, the cursor will stop at the location of the old
item. Even deleting the text around it will not solve the problem.

Does anyone know how to eliminate these ghosts?

Do you have Track Changes enabled? This can cause such problems until you
have accepted the changes.
