Field1 does not pull data from field2

I have written forms in Outlook 98.
Now they have been updated to Outlook 2003
When you click on Send on a response form you get "Operation Failed"

Now I have tried rebuild the form using Outlook 2003 but the code will not
work.
Left([Employee First Name],1)&[Employee Last Name] Is suppose to populate
the Username field with ie JDoe for John Doe

[Username:]&";"&[Manager/Director] Is suppose to populate the TO: Field

I checked for fixes in the KB but they do not work.

It worked fine till the upgrade.

Any help would be appreciated.
