FieldListCtrl Listbox

I have downloaded Lebans fieldlist sample and tested it out within his
access program of which it works there. What I am trying to do is find out
how to create it from scratch. When I check on the properties of his field
list control, it shows for the class:

ACCWIZ.FieldListCtrl.1.8

and for the OLE Class:

FieldListCtrl.1

I have tried looking under 3 different locations using Access 2002, List of
References, Additional toolbox list, and Object Browser.
