Fill In Field for Template Forms

I use the Fill In field when creating forms for my company. We use the Fill In fields to pop up boxes requesting information for a certain field such as Customer Name.

The problem that I am having is formatting the fields in such a way that the correct boxes will appear in a specific order, currently we have to enter the Customer name then the Unit Amount rather than the Customer Addres.

How do you manage the order the Fill In field boxes appear?
