Fill In Form Publisher 2007

I am new to Publisher & am working on building a website, so far so good.
What I can not figure out is how to create a fill in form that can be
submitted back to me.

I tried creating it in Word & also converted it to a PDF. Problem is that
when you click on the link, the form is all screwy on the screen.

HELP!
