"Fillable" document or template

I recently received from a government official a "fillable" Word document.
That is, throughout the document there were spaces where I could fill in such
things as Name, rank and serial number without altering the balance of the
text. It worked just fine and I could send back to him via Email the
completed form.

I'm using Word for Mac 2004 Ver 11.2, and cannot find anything in Help under
"fillable documents" or "templates". I'm probably not looking in the right
place, or it might require a plug-in or other program of some kind.

Whatever help or suggestions you can offer, I'd appreciate very much.

Hi Dave,

you've already used the correct name in your question; see if Word's help
feature gives you details on "forms". You can use the Forms toolbar
(View>Toolbars>Forms) to create your own "fillable" documents.
