Fillable Forms

I'd like to send out an email to collect data from summer school applicants
(as an example.) Is there a way to collect all the data from the 200
respondents who each return (as an email attachment) an MS Word "form" that
has been filled out by each of the respondents?

(Or is there a simpler way?)

and - is there a site (or newsgroup) that pertains specifically to the use
of MS Word to make fillable forms?

Any help will be greatly appreciated.
