Fillin ASK field question

I want to create a document, press a button (macro??) and it comes up and
asks you 3 different fields. I used to know how to do this (about 8 years
ago.....) but I have forgotten how to. I would also like it to save the
document as filename_xxxxx. DOC where xxxxx is one of the fillin values.

There will be several of the same fields though out the document.

The gray matter is steaming! It has been too long!

This is Word 2007, so menus are deffiently different...
