Filter by form in a preformatted form

I am presenting the user data in a pre-formmated form.
However, Ineed the user to be able to use the filter by form or filter y
selection option.

For some reason it does not seem to work. I just get my own form with no
possibility of actually enterin anything.

What am I doing wrong?
