Filter_by_form question

To further refine my prior request:

How do I force the user to choose filter_by_form when they
open the form? Simply, one of the fields is a location
number and I want them to pick their own location, so the
form only shows them their records.

I assume the ideal is a pop-up box on the open event that
will ask them to enter a parameter and then filter by it
on opening the form. Can someone show me the code?
