FIM-A and Postnet commands gone

I don't know when they left, but I can no longer print the FIM-A and PostNet
codes on envelopes in Word 2004.

I'm running Word 2004 Version 11.1 and OS 10.4

The "envelopes" dialog box used to have check boxes to turn the codes on and
off, but they are no longer there.

Any ideas on how to get them back?
