footers, predefined, adding new & deleting old

I want to add a footer including date (left), path & file
name (centre), and "Prepared by ....." (right). This
footer I would like to make as a permanent footer on all
worksheets (new & old). The problem I am experiencing is
that it doesn't end up in the predefined footer selection
box (as indicated in my Excel book)to be selected again
and again. What I have to do is recreate this footer each
time I open a worksheet.

The predefined footer box has about 12 or 15 footers that
I would prefer to delete but can't. Is the problem
because the number of footers in this box is already at
its limit, or is there another way aroudn this problem?
