Footnote Numbers Do Not Restart on Each Page

I'm using MS Word 2003 with a 217-page document consisting of tables.
When I print (to a .pdf file), footnotes up to page 156 are numbered
consecutively and on later pages the numbers restart on each page. I
would like numbers to start on each page.

Incidentally, I print in reverse page order.

When I edit the document in Word, sometimes the numbers switch between
consecutive numbering and restarting on each page. As I watch the
screen, a footnote number 1 changes to 173 and then back to 1. It's
entertaining in its pesky little way. Most of the time, however, the
footnote numbers are as I wish them to be, starting over on each page.

The problem occurs when Word prints.

I have tried several times to go to the beginning of the document, go
Insert-Reference-Footnote, and Apply the setting to restart numbers on
each page for the whole document.

I have tried to disable some of the print options: field updates,
background printing. No success yet.

My next steps are:

(1) Reboot my machine.

(2) Select the entire document (text not footnotes) and apply the
footnote settings (instead of using the dialog box to apply settings to
the whole document). Then I'll select the entire document and view the
footnote settings. I hope they will be as I wish. If not, I'll try step
3 below.

(3) Check and correct the footnote settings section by section. Since
the problem is in about 60 sections, I hope to avoid this.

(4) If this doesn't work, I'll have to take the document apart (a bit
of a shame, since I making final edits when the problem occurred).

If you have read this far, thank you. I wonder if you think there's
something else I should be looking at. Many thanks for any suggestions.
As you can imagine each attempt to correct the problem takes
substantial time.

Click Footnotes (Location)
Set 'Start at:' to 1 (Format)
Set 'Numbering' to 'Restart each page' (Format)
Set 'Apply changes to:' to 'Whole document' (Format)
Click 'Apply'
