Footnote reference numbers are incorrect

There are times that I find that footnote reference numbers appear in
incorrect sequence -- i.e. the associated footnote refers to a different
footnote reference number than the one that appears on the screen (or hard
copy).

Sometimes I also find that a blank line is inserted above a footnote that
for some reason can't be removed -- i.e., when I try to delete it WORD
returns an error message about an illegal operation.

Does anyone have a solution for these?

Thanks.

(BTW, the mistaken reference numbers problem is not due to mixing
'automatic' and 'manual' footnote numbers -- all footnotes were created via
the Reference menu option.)

Are any of the footnotes in tables, frames, or text boxes?

They're usually in tables. What's the connection there?
