Form blank after sending

I created a form, and when I go to Tools, Forms, Choose Form, I see the form
I created with the fields I can enter data into. After entering data then
"sending" the form, the "received" form is a Blank version of the form I
created. It is missing the data I just entered.

Any idea why this is? Did I miss a step when creating?

Did you bind the controls in the form to user properties so the data is
preserved? Are the controls and user properties being preserved? See
