Form Checkboxes in a Table

Hello all. I am not sure if I am in the right area or not,
but I really do need some help. I am designing a form that
contains checkboxes and I'm positioning these checkboxes
within a table. I have seen the code that enables one to
put checkboxes within a frame, but because I have several
checkboxes in various positions on a page, have elected to
position them using a table, which work better.

Now, I know that the checkboxes have bookmark names, but
am not savvy enough to write the code that would prevent
more than one checkbox in a group of two, three, or four
checkboxes from being selected. The customer does not want
to use the drop-down list because if does not display the
content until you click it.

These checkboxes will be presented side-by-side on a page
like:

Technology: Surface Mount () Thru-Hole () Mixed ()

Only one of the three above may be selected....

I hope I've presented this in a clear fashion, as I do
need some help with just some basic code that will help me
get started...

Thanks for the reply Jeffrey. I have seen this write up, and it's quite good
for those that wish to use frames to enclose checkboxes. Frames however do
not allow the formatting flexibility of tables, and a frame cannot be placed
within a table.
