form crashes Word 98

All I wanted to do is create a form for people to fill-in
electronically.

It's full of check boxes and text fields and all that, but it crashes
my Mac running OS 9.2.2, and other computers running OS 10.3.

It's only the protected version of the form that crashes, my
unprotected, editable form is OK.
Obviously something is corrupted, but I can't figure out how to fix it
(I've tried starting from scratch, using less fields, etc.)

Is there a way to have the user enter text without using text fields,
but then can I have them use check boxes and drop-downs elsewhere
within the same document?

I'm creating the form on a Mac G4 running OS 9.2.2 and Word 98. It will
be used by a number of people running all combinations of operating
systerms (Word, 2001, XP, Panther, etc.) I need the final result to be
a Word file of some kind (not html).
