Form created in Outlook 2000 not working

I have a vacation request form created with Outlook 2000.
The user fills in the form then sends the request to their
supervisor. When the form in opened in Outlook 2003,
filled in and the Send button in clicked, the mail sits in
the Outbox and never gets sent out. Any help would be
appreciated.
